NOTICE: CHANGE OF CUSTODIAN FOR HEALTH RECORDS

Effective February 1, 2025

 

Please note that Ideal Physio will become the custodian of your health records/chart notes that have been previously managed by Bowen Medical Group.  

 

Your care will not be affected as your current practitioners will be making the move as well and you can be assured your records will continue to be handled securely in compliance with the Personal Health Information Protection Act (PHIPA).

 

You retain the right to access or request corrections to your records by contacting Ideal Physio at 289-266-3620.
